Transliteration
om adya tithau purvokta evam guna-visheshana vishishtayam shri mahavishnor ajnaya pravartamanasya adya brahmano mama sakala papa kshayartham shri parameshwara preetyartham
Meaning (English)
Om — today, on this tithi, characterised by the above-mentioned qualities, set in motion by the command of Lord Maha Vishnu, during the current age of Brahma, for the dissolution of all my sins and for the pleasure of the Supreme Lord (intent of vrata/karma stated here). Universal sankalpa formula used before any Hindu ritual to declare time, place, intent, and divine sanction.

Sit before deity. Take water in right palm. State date, place, intent, divine sanction. Pour water on ground while completing sankalpa. The water symbolises the irrevocability of the declared intent — once made, the karma must be completed.
